Other Healthcare Activities in Germany industry analysis

On average, the industry recorded an annual increase in turnover of 2.5% between 2020 and 2025. In January 2023, the list of diagnoses was expanded, allowing doctors to prescribe long-term remedies for more illnesses without fear of budget restrictions and possible recourse claims. This is likely to have a positive impact on the utilisation of services and turnover for companies in the sector in the long term. In addition, prices for physiotherapy and physiotherapy treatments increased. However, these increases in turnover were not enough to offset the inflationary price pressure caused by rising wages and material costs, which is why personnel and material costs are likely to continue to rise.

Trends and Insights

  • Without a doctor's prescription, patients cannot receive remedial therapy under the statutory health insurance system. An alternative would be direct access. By giving patients direct access to remedial therapy, the quality and cost-effectiveness of patient care could be improved.
  • With a growing proportion of people aged 65 and over, the consumer market is likely to soon account for the largest share of sales. As people get older, the risk of diseases such as strokes and chronic illnesses increases.
  • As the most populous region, the centre region has the highest proportion of companies in the sector. North Rhine-Westphalia stands out here, with the highest number of companies per federal state in Germany as a whole.
  • Physiotherapists are not allowed to advertise a specific treatment or product. In competition, factors such as the practice location or free parking spaces as well as a good reputation therefore play an important role.

Industry Statistics and Trends

Market size and recent performance (2015-2030)

Industry revenue has grown at a CAGR of 2.5 % over the past five years, to reach an estimated €38.0bn in 2025.

Trends and Insights

The shortage of skilled workers jeopardises prompt patient care in physiotherapy

  • The shortage of specialists in physiotherapy is now so severe that it is jeopardising the timely care of patients. On average, it takes a practice more than eight months to fill a vacancy. According to the Federal Employment Agency, the sector continues to be one of the occupational groups most affected by the shortage of skilled workers and has had a score of 2.7 out of a maximum of 3.0 on the shortage scale since 2022. As fewer and fewer specialists are coming in and the workload is increasing, many practices are unable to accept all therapy requests, have long waiting lists and have to deal with overwork and even sickness-related absences in their teams. This leads directly to lost revenue, even though there would be sufficient demand.

Trends and Insights

The ageing population is exacerbating staff shortages in therapeutic and emergency services

  • The ageing population is a key driver of the increased demand for medical care providers such as physiotherapists, occupational therapists, speech therapists and podiatrists. According to recent reports by the scientific services of the health insurance providers AOK and Barmer, almost 60% of all treatments for therapeutic products are provided to people over the age of 65. With the baby boomer generation coming of age, a further significant increase in chronic illnesses, multiple complaints and the resulting need for therapy can be expected from 2030. The average number of therapy units per patient will also rise with increasing age, particularly in occupational therapy and physiotherapy. At the same time, the requirements for barrier-free practices and care close to home are becoming increasingly urgent as the number of patients with limited mobility grows.
