Hospitals in the UK industry analysis

Over the five years to 2025-26, hospitals' revenue is expected to grow at a compound annual rate of 0.4% to £127.4 billion. Hospital services are currently being stretched to the limit as they struggle to navigate financial constraints and workforce shortages, while appointment waiting times remain high. Hospitals still face long elective backlogs, reaching 7.36 million in March 2025, according to NHS England. However, the growth in private hospitals is fuelling industry revenue as patients are fed up waiting for treatment. Parallel to this, the government raised funding for the NHS in 2025-26, announcing a £29 billion real-terms funding increase in June 2025, supporting modest industry revenue growth. Industry revenue is projected to grow by 3.5% in 2025-26.

Trends and Insights

  • NHS Hospitals struggle with inflationary pressures, staff shortages and growing demand. Hospitals are struggling to clear elective backlogs due to workforce shortages and a lack of real-term funding growth.
  • A surge in scheduled treatments strains hospital bed capacity. High demand for planned and routine procedures has led to a considerably high bed occupancy rate, challenging the safe threshold in hospitals nationwide.
  • London’s array of universities attracts companies to the region. The capital’s connection with teaching institutions like King’s College, University College London and Queen Mary fuels a vibrant research sector, boosting the appeal of hospital set-ups in London.
  • Internal competition shapes the Hospitals industry. As private sector participation grows, hospitals must remain competitive in terms of price, quality, facilities and location, thereby increasing competitive pressures within the industry.

Industry Statistics and Trends

Market size and recent performance (2015-2030)

Industry revenue has grown at a CAGR of 0.4 % over the past five years, to reach an estimated £127.4bn in 2025.

Trends and Insights

An ageing, expanding and growing population, coupled with inflation, has damaged real-term NHS revenue growth

  • Government funding determines NHS hospital budgets for the year, so it forms the foundation for the hospital industry’s revenue, with volatility and growth prospects closely tied to these budget decisions. NHS England has a planned budget of £215.6 billion in real terms in 2025-26, according to The NHS Confederation. However, due to constraints and cost pressures, this budget is still £6.6 billion short for 2025-26 according to NHS England, underscoring the challenges of balancing rising demand with available resources. Given recent pay settlements for consultants and junior doctors, pay will be a big cost pressure for hospitals. Wage costs are likely to eat into revenue in 2025-26 as hospitals depend on staff to work longer hours and hire temporary healthcare professionals to plug workforce shortages.

Trends and Insights

New hospitals ease capacity issues, but short-term strains persist

  • The New Hospitals Programme, the last government’s initiative to build 40 new hospitals in the 10 years through 2030, was under review by the Labour Government due to a lack of funding. The result in January 2025 is that the programme will go ahead, backed by a £15 billion investment over consecutive five-year waves and there will be 23 projects starting between 2025 and 2035, according to the Department of Health and Social Care. As of July 2025, two new hospitals had been opened: Midland Metropolitan and Dyson Cancer Centre. This will free up capital investments and hospital infrastructure improvements moving forward.
